The letter from Beverley Hughes clarifies the urgent action that should be taken by each LSCB responsible for Serious Case Reviews (SCRs) judged inadequate by Ofsted, and with respect to new and future SCRs. The accompanying guidance note, SCRs Action note, sets out the Government's expectations of the immediate action now needed to improve the integrity and effectiveness of SCRs to provide a more effective basis for making necessary improvements to local safeguarding arrangements, particularly in respect of the chairing arrangements for SCR Panels.
