The CAF form is designed to help practitioners record and, where appropriate, share with others, their assessments, plans and recommendations for support. The form (updated February 2008) is available as a pdf and in an editable word document format so it can be downloaded, and completed.
The CAF form is now available in Polish, Lithuanian and Portuguese (November 2008).
NOTES
The CAF form can be downloaded, printed and photocopied without permission, but
the framework and content of the form must not be altered in any way.
The pre-assessment checklist can be used by practitioners as an aid in deciding
whether a common assessment is appropriate.
The guide to definitions used in the CAF form gives a general description of signs to look for in relation to each of the three elements of CAF.
