Transition Support

The Aiming High for Disabled Children review found that more needed to be done to co-ordinate services for young disabled people in transition to adult life, and to ensure young people and families can access high quality information at key transition points.  

The review allocated £19 million to a Transition Support programme, which will address these specific issues. It will seek to have a similar impact on services for disabled young people as that which the Early Support programme has had on services for younger disabled children. An announcement on the Transition Support programme will be made later in 2008.

The Transition Information Network (TIN) is an alliance of organisations and individuals who come together with a common aim: to improve disabled young people's experience of transition to adulthood. The TIN Reference Group has helped to inform the development of the Transition Support Programme.
